Don't have a great deal of information on this. You had to fill out a request and it was mailed out from I believe "The Sporting News" shortly after Pete broke the hits record. It is still in the original plastic wrap that it arrived in.
Probably Pete's first gu card.

Any additional information would be greatly appreciated.
